How the "Quick Lookup Service" Works
1 Find an entry for a Chicago or Cook County birth, marriage, death, or Catholic church record in an online, microfilm, or
microfiche index. If you don't have access to the microfilm or fiche indexes, I can send index pages.
2 Submit a
lookup request using the form on my website and watch for my invoice email to arrive. I do lookups on Tuesday, Wednesday, Thursday, and Saturday and you should hear from me within a few days of submitting your request. If you don't, email me immediately.
3 Send lookup fees by PayPal or check after you receive my invoice. The fee for each lookup is $5.00.
4 Watch for my "payment received" email. It will include a link to a digital image of the record, scanned from microfilm, and I will mail you out a laser printed copy of the record the next day.
