The early
certificates are indexed in the Cook County Birth Index, 1871-1916. There is no
public index available for 1916-1922, however it is possible to find a
certificate on these microfilms without a certificate number if the birth month
and year are known.

Certificates from 1878-1894 are arranged numerically by a certificate number that can be found in the Cook County Birth Index, 1871-1916. Certificates from 1916-1922 are arranged alphabetically.
